To file your pagibig salary loan, just accomplish MLAF (Multipurpose Loan Application Form) and fill up all necessary details.
You can file it through online via pagibig virtual account or you can go to the nearest pagibig branch in your area and bring the following in photocopy.

2 valid id's with 3 signatures
LOYALTY CARD
SELFIE Holding your ID and loyalty card ( if filed through online )
